Anecdotally, my nephew played D1 college soccer and suffered concussions.

I had two concussions playing rec ball at Mike Rose over the last 6 years. One during a corner kick where I got punched in the head by the keeper(accidentally) and another knocking heads with a guy on a header. I dont know how you get any of those out of the game without drastically changing the game.
